甲状腺癌病因 大多数临床上遇到的分化性甲状腺癌(乳头***状腺癌和滤泡状癌)病因尚不明确,讨论最多者有二,即放射线 与流行性甲状腺肿。髓样癌和未分化癌的致病因素亦不清楚,已知有些髓样癌有家族遗传性,部分未分化癌可能来 自分化性乳头状腺癌和滤泡状癌。有些甲状腺的恶性肿瘤可能是 Hashimoto 甲状腺炎的恶变。 ( 1 )放射线 已知放射线能导致某些肿瘤的产生。从理论上讲一个长期暴露在放射线下以及凡是能促使甲状腺滤泡细胞生长的因 素包括甲状腺叶切除、碘缺乏和抗甲状腺药物都是甲状腺肿瘤的致病因子。 ( 2 )地方性甲状腺肿与甲状腺癌的关系很密切。 ( 3 )实验室的甲状腺肿瘤 放射线和 131I 对鼠类在应用硫脲类药物促使 TSH 增加时是一种致病因子,但持续刺激的时间要长达实验动物的半生。 甲状腺癌诊断方法 异常的甲状腺肿大包括癌肿、腺瘤、甲状腺结节肿、甲状腺功能亢进、亚急性和淋巴滤泡性甲状腺炎等。有时几种 疾病混杂在一起。临床上对典型的甲状腺肿瘤良、恶性容易鉴别,但腺内型的甲状腺癌特别像分化好的滤泡状癌, 和腺瘤十分相似,单靠临床鉴别显然有困难,还需作其他辅助检查。少数甲状腺结节肿、亚急性甲状腺炎、淋巴滤 泡性甲状腺炎、 Riedel 甲状腺炎与甲状腺癌也颇难区别。由于治疗方法不同,所以必须仔细询问病史和体格检查,要抓住其中要点,再结 合相关的检查,综合分析,以得出较为可靠的临床诊断。 ( 1 )病史和局部体检 异常的甲状腺肿大或结节首先要和肿瘤作鉴别,病史和体检对鉴别诊断常有很大帮助。体检发现甲状腺弥漫性肿大 或具轻度结节肿,如再作 131I 扫描和吸碘率试验,一般就可确诊。如先有上呼吸道感染史,再作血沉、蛋白电泳、 131I 等检查以确诊,这里须强调检查要及时,否则就失去诊断价值。如中年妇女甲状腺结节样肿大,伴有轻度甲状腺功 能亢进或减退的症状,部分患者伴有神经官能症;甲状腺明显肿大,两叶可以不对称,腺体表面呈多个半球形突起 ,有时锥状叶也肿大;整个甲状腺的外形酷似旧式的笔架,边界清楚,轮廓分明;其质地实而富有弹性;颈淋巴结 不肿大,要考虑为淋巴滤泡性甲状腺炎(又称Hashimoto 甲状腺炎),可作 TGA 、 MCA 、 T3 、 T4 、 TSH 等放射免疫测定;必要时用细针穿刺作细胞学检查,以求确诊。如果患者有多年的甲状腺结节肿史,病程慢,症状 不明显,颈淋巴结不大,且患者来自甲状腺肿流行区,有关的检查也在正常范围内,则患甲状腺结节的可能性较大 。如果部分结节在短期内迅速增大,则要考虑可能为恶变。如是老年妇女,甲状腺虽不大,但质地很硬,边界不清 ,与气管紧密固定,有时甚至有呼吸压迫症状,要考虑为木性甲状腺肿( Reidel 甲状腺炎)。但要排除癌肿,手术探查还是有价值的。术中确诊非癌肿后即不必作甲状腺全切除,可切除甲状腺峡 部,以解除呼吸压迫症状。 在初步排除非肿瘤的甲状腺疾患后,应确定甲状腺癌。一般说年龄是一个重要的参考因素,年幼的比成年人的甲状腺孤立结节恶性比例要大, 15 岁以下患者的甲状腺单个结节中 10%~50% 是恶性,但都是分化好的甲状腺癌。中老年人的甲状腺癌发病率也很高,特别是未分化癌大多在 60 岁以上。性别与各种病理类型甲状腺癌的关系也很密切,其中乳头状腺癌特别好发于青年女性。 ( 2 )放射性核素检查 放射性核素检查可以明确甲状腺的形态和位置以及甲状腺和甲状腺肿块的功能,所以该项检查已成为诊断甲状腺疾 病的常规手段。根据甲状腺结节的吸 131I 或 99mTc 的功能一般可将其分为 4 类:①热结节。②温结节。③凉结节。④冷结节。 甲状腺癌一般单个冷结节为恶性肿瘤的可能性较大,根据中国科学院肿瘤防治所的资料,冷结节中癌肿发现率 为 54.5% 。当然冷结节也不一定都是癌,其他良性疾患也可出现此图象,还应结合病史、体检和其他有关检查,综合分析才 能作出临床诊断。

Buckling Under the Malaise of Detroit TWO cars were left in the three-car showroom of Larry Friedman’s Chevy dealership, and one of them wasn’t for sale — his baby-blue 1954 Bel Air with the dice hanging from the rearview mirror, a pristine memory of breezier times in the auto trade. The salesmen’s desks sat empty, and a half-full box beside one held giveaway calendars advertising a business about to close for good after more than 80 years.The end came quickly here at New Norris Chevrolet, an echo in New Jersey of a collapse in Detroit. In December, Norris sold 12 cars, half its previous monthly average, and when the numbers weren’t any better by the end of January, Mr. Friedman couldn’t see a way to make them work anymore. He called his employees — many of whom had been hired by his late father, Mitch — into the showroom one morning a few weeks ago to tell them that Thursday, Feb. 26, would be the last day.“I choked up a little bit,” said Mr. Friedman, 42, who started working at Norris as a boy and took over after his father’s death in 2005. The license plate on his Bel Air looked ahead to his potential successors, “3 Boys,” referring to his sons. But one of Norris’s service advisers had a stroke that weekend. “So that Monday he said, ‘It’s over, get back to work.’ ”Larry Friedman moved in 2001 from service to the sales floor, where he sometimes parked a vintage Chevy, like his Bel Air, that triggered warm recollections from customers about first cars, summer nights and the glory days of General Motors. Norris sold about 325 new cars, and 100 used, in a good year. But last year was not a good year: Gas prices spiked; interest rates on leases soared; credit dried up, for buyers and dealers both; uncertainty about G.M.’s future made buyers wary. And then Detroit started asking for more money: higher interest rates on the inventory dealers held; more charges on cars that sat unsold.“The economics of the business changed overnight,” said Jim Appleton, who has watched many New Jersey dealerships struggle to finance their operations. “They’re essentially collateral damage in the credit crunch.”Mr. Friedman’s desk looks across the showroom floor and past his Bel Air toward a wall of photos he will soon have to box up and take home, among them one of his father shaking hands with Mickey Mantle at a benefit for the American Cancer Society.
