Osseous surgery (also called flap surgery with bone reshaping) is a periodontal surgical procedure used to treat advanced gum disease (periodontitis) that has damaged the bone supporting the teeth.
Osseous surgery is performed when deep periodontal pockets form around teeth due to infection and bone loss. The procedure:
Removes infected gum tissue
Eliminates bacteria and tartar buildup deep below the gumline
Reshapes uneven or damaged bone
Reduces pocket depth so gums can heal properly
The goal is to stop disease progression and preserve natural teeth.
Why It’s Needed
In advanced periodontal disease:
Bacteria destroy gum tissue
The supporting bone becomes irregular or develops defects
Deep pockets (usually >5–6 mm) form
Regular scaling and root planing are no longer enough
